I understand that it is my responsibility to complete a Request for Alternate Media form for each form of media that I require in alternate format. I am aware that, although NIACC Disability Services will make every effort to obtain the alternate format requested as quickly as possible, it may take time. I understand that any alternative text that is supplied to me is solely for my own educational purposes and I cannot reproduce or distribute the alternative text provided to me. As soon as I am no longer the owner of said media, the alternate media provided to me will be destroyed.

Note: It’s difficult to predict the time frame it will take a particular publisher (or other provider) to supply the alternate media. If you’re concerned and know you will need the media more quickly than it may be provided, please deliver a syllabus that contains the order of information needed to NIACC Disability Services so that portions of the media can be created in an alternate format as required by your class schedule.
